Thank you for choosing our Boarding Facilities! Our Boarding Facilities will allow your pet to be as comfortable as possible while you are away.
PLEASE NOTE: We will NOT accept submissions for this form without a boarding reservation scheduled with our reception staff via phone call or email. If you have booked a reservation, please make sure to submit this form at least 48 hours before the drop off date in order to finalize your booking. Failure to submit this form may result in a cancellation of your boarding reservation.

Drop Off: 3:00 p.m. – 5:00 p.m.
SATURDAY
Pick Up: 8:30 a.m. – 10:00 a.m.
Drop Off: 11:00 a.m. – 12:00 p.m.
PLEASE NOTE: Clients who do not arrive during the time windows specified above will not be able to drop off or pick up their pet on that same day. Reception staff will reschedule you to come in the next day instead.
There are NO drop-offs or pick-ups during our emergency hours or holidays as we don’t have the staff to accommodate drop-offs or pick-ups during these hours.
Pets are only released to their owners registered in our system, unless prior arrangements have been made. Who is authorized to pick up your pet?

BOARDING MEDICAL REQUIREMENT POLICY

I have read and understand the boarding medical requirement policy.
BIRDS:
• Must have been tested and confirmed negative for the common viruses (Polyomavirus, Beak and Feather Virus) and bacteria (Chlamydia), within the past one year.
RABBITS:
• Must have RHDV vaccine updated annually.
FERRETS:
• Must have Rabies and Distemper vaccine updated annually.
If you have any questions regarding our boarding requirements, please call reception to inquire.
Is your pet on any medication or supplements?
List the name of the medication and instructions per day.
There is an additional medication fee per day. Please ensure you bring enough medication to last for the duration of your pet's stay. If your pet's medication was prescribed by another veterinary clinic, please let reception know before your boarding reservation.

We do not have the following accessories in the clinic, so you must provide these for your pet.

• Other accessories (toys, wheels, hideouts, etc.)
ADDITIONAL INFORMATION
Use the space below to inform of us of any additional information and/or instructions regarding your pet.
PLEASE NOTE: Our kennel staff will try their best to accommodate special requests for your pet. Please keep in mind that, in addition to their regular hospital duties, our kennel staff are caring for many boarders at a time. Special requests such as e-mail updates or photos of your pet during their stay may not be possible. Additional fees may be applied for hand-feeding, medicating and grooming. Please notify our reception staff if your pet requires special attention. While you are away, we can do an examination and wellness test on your pet. Grooming and examination appointments will be arranged based on availability at the time of booking, but may not be possible during your pet’s stay.
PERSONAL ITEMS FOR BOARDING PATIENTS
Your pet having his/her personal belongings (toys, treats, blankets) while boarding is important to The Links Road Animal & Bird Clinic and your pet. Being away from the family is not always easy on our little ones and having a touch of home with them can be comforting. However, there are special considerations to make when your pet is boarding. While your pet may have a special bed, blanket or toy that they would never chew up at home, they may decide to chew it up while in a kennel away from home. We ask that NO irreplaceable/meaningful/non-washable items be left with your pet while they stay with us.
Please read and sign the following waiver when leaving any personal items for your pet:

• I understand that The Links Road Animal & Bird Clinic is not responsible for loss or damage to personal items left with my pet. I understand that personal items may not be returned in the same condition in which they were left and some personal items may be damaged to the point of being discarded.
• I understand that I am not to leave any irreplaceable or non-washable items.
• I understand that leaving personal items with my pet could pose a serious health risk if they were to ingest them.
• I understand that the veterinarians have the right to withhold any personal items from my pet if they foresee a health risk.
• I understand that my pet may be more prone to accidents or inappropriate elimination while boarding and that kennel staff may not be able to launder my pet’s bed depending on their size and/or materials. When leaving a bed with my pet, I understand that the bed may become soiled and do not hold The Links Road Animal & Bird Clinic responsible for loss, damage or replacement.

I understand that the Axelson Veterinary Professional Corporation, operating as The Links Road Animal & Bird Clinic, has a Personal Information Policy in accordance with requirements of the Personal Information Protection and Electronic Documents Act.
By checking the box below, I am consenting to the collection, use and disclosure of my personal information in accordance with the purposes set out in the policy, which include the following:
• Maintaining complete and accurate client files, and complying with the requirements of the College of Veterinarians of Ontario, the Veterinarians Act and regulations under the Act;
• Providing goods and services to veterinary clients, including contacting clients to schedule appointments and follow up on patient treatment, billing for goods and services and notifying clients about new services and promotional offers; and
• Communicating and working with third parties providing veterinary medical or other services to clients, including other veterinary facilities and insurance companies which may pay for all or part of the cost of such services.
I understand that:
• My personal information will not be used or disclosed for purposes other than those for which it was collected, except with my consent, or except where the use or disclosure is required by law;
• I have the right to view my personal information and have it amended, if inaccurate or incomplete; and
• A copy of the Policy will be provided on request.
Our Photo Release Policy

I hereby grant The Links Road Animal & Bird Clinic the right and permission to use, re-use, publish and re-publish images of my pet in the following formats:
• Publications (newsletters, brochures, projects, etc.)
• Press/media releases (online and printed articles and announcements)
• Advertising materials
• Our Website
• Social media platforms (Facebook, Twitter, Instagram etc.)
By signing this consent, I acknowledge that, for valuable consideration, I hereby release and forever discharge The Links Road Animal & Bird Clinic and the owners and employees from and against any and all claims, damages or suits which may arise from the use of the photographs, videos or likenesses for The Links Road Animal & Bird Clinic, including, but not limited to, the exhibition of the above-named pet’s photograph or likeness or publication of the pet’s name.
